"a fractional CTO never has the same mandate as a permanent CTO, and will always need buy-in from the ones that hired him/her, the founders, the board" - this applies equally to a permanent CTO, who will also need buy-in from the founders and the board.
I would guess that the main risk a fractional CTO faces that a permanent one doesn't, is being perceived as not having the same commitment to the company.
